HM Insurance Group provides administrative services through its member companies, HM Life Insurance Company and HM Benefits Administrators.

Coverage is underwritten by HM Life Insurance Company, Pittsburgh, PA, in all states except New York under policy form series HL100, HL300, GP292, GP595, GP895, GLC-102, HG403, GP197, GP190, HL900, GP198, HL601, HL902, HL904, HGCA 5000, HGCA 6000, HGCA 8500, HGCA 9000, HGCA12000, HM905, HMPEL1105 or similar.  In New York, coverage is underwritten by HM Life Insurance Company of New York, New York, NY, under policy form series NY GP292, GLC-102, GP198, GP895, HL100, GP197, HL900, HG403, DBL-88, DBL-89, HL601, HL902, HM905 or similar.  HM Life Insurance Company and HM Benefits Administrators provide certain administrative and customer support services.  The coverage or service requested may not be available in all states.
